Becoming a Patient
Your first visit, from 1 hour to 1 1/2 hours (if acupuncture is included), you and Dr. Tack will review a comprehensive intake form, over-viewing organ systems and previous health patterns, followed by an oriental medicine based physical evaluation. If you bring the following to your first appointment it will allow us to get more accomplished. All medications/supplements, recent lab work, and for involved cases a timeline. Together we will prepare a plan that is personalized to fit your schedule, health care requirements, and finances.
We do offer an initial complimentary 15 minute session as requested for those interested in predetermining if your condition/needs are a good fit with our clinic.
