If you are unable to print these PDFs, please call our office manager at (360) 683-0652, and we will send you an introductory packet by fax or mail. Otherwise, please arrive 20-30 minutes early for your appointment to allow time to complete the necessary paperwork.

2. If you are taking supplements or other medicines it would be helpful to  bring them to your first appointment.

3. If you have copies of your most recent lab work, please bring a copy to the visit.

4. It may be helpful to write down your questions and concerns prior to the visit to ensure that they are addressed.

5. Creating a time line is appreciated.
